Background: Epiploic appendagitis (EA) is a rare and self-limiting cause of acute abdominal pain, often mistaken for conditions such as appendicitis or diverticulitis due to its overlapping clinical presentation. This case series highlights two distinct instances of EA encountered at a single institution, emphasizing its diagnostic and therapeutic nuances.

Methods: The first case involved secondary EA in a 54-year-old male with acute appendicitis, necessitating surgical intervention. The second case described primary EA in a 73-year-old male, successfully managed conservatively with non-steroidal anti-inflammatory drugs.

Results: Both cases underscore the role of computed tomography as a gold-standard imaging modality for diagnosing EA, characterized by hallmark findings such as inflamed fat-density structures and the hyperattenuating ring sign.

Conclusions: By delineating primary and secondary EA, the report aims to increase awareness among clinicians to distinguish this condition from other acute abdominal pathologies. Early recognition of EA and its benign course can prevent unnecessary surgical interventions, optimising patient outcomes and reducing healthcare costs.
