Background: Gastric atrophy (GA) is a pre-neoplastic lesion in the development of gastric cancer (GC), progressing through the Correa cascade from chronic gastritis to intestinal-type gastric adenocarcinoma. Despite this known pathway, the exact risk of GC development in individuals with GA remains uncertain, complicating decisions regarding the need and intervals for surveillance.

Methods: This systematic review was conducted in accordance with PRISMA guidelines, aggregating studies that evaluated the risk of GC in individuals with GA.

Results: We included 18 studies with a total of 25,455 patients. The relative risk (RR) of GC in patients with GA was found to be 15.1, with a 95% confidence interval of 13.5 to 16.9, based on standardized incidence ratios. This risk is comparable to that observed in non-dysplastic Barrett’s esophagus.

Conclusions: Our findings suggest that GA significantly increases the risk of developing GC, possibly more than previously recognized. However, further large-scale studies, particularly in Western populations, are needed to more precisely quantify this risk and inform surveillance strategies. Future research should standardize the assessment of GA, including factors like Helicobacter pylori status, GA distribution, and grading.
