Background: Nail gun injuries to the hand are a common encounter amongst those in the construction industry and a frequent presentation to the emergency department. Despite the large number of nail gun injuries occurring daily, there are few reports on the impact of nail gun trauma to structures such as nerves, vessels, tendons and bone at the hand and wrist. A systematic review was conducted to analyse morbidity and functional outcomes following nail gun injuries to the hand.

Methods: A literature search of four scientific databases was performed to identify papers describing nail gun injuries to the hand, up to and inclusive of October 2023. In total, 16 studies were eligible for inclusion, capturing 185 nail gun injuries to the hand.

Results: In the 16 studies included, there was 1 major vascular injury, 23 nerve injuries, 46 tendon injuries and 36 bone injuries. Morbidity on follow up included 2 cases of motor dysfunction, 23 chronic pain or paraesthesia, and 11 left with reduced range of motion.

Conclusions: Nail-gun injuries to the hand require thorough evaluation techniques, understanding of surgical versus nonsurgical management, and close post-operative monitoring and rehabilitation. This is an evidence level IV paper.
