Background: Genicular nerve block is a successful regional anaesthesia technique in the management of acute and chronic knee pain secondary to arthritis and post-knee surgery. It is also being used for acute pain management in tibial plateau fractures. We report the novel use of genicular nerve block for tibial plateau fracture repair.

Methods: A 61-year-old patient presented with acute 9/10 left-sided knee pain secondary to a left tibial plateau fracture due to a mechanical fall. An ultrasound-guided genicular nerve block was administered perioperatively to the superior medial, superior lateral, and inferior medial genicular nerves. 1% lidocaine and 20 mL of 2.5% levobupivacaine were administered under ultrasound guidance.

Results: The patient reported a postoperative pain score of 0/10 at rest and while moving her leg for the next 24 hours.

Conclusions: Genicular nerve block is a safe and efficient technique for providing analgesia in tibial plateau fractures peri- and postoperatively. It is especially useful in reducing the need for opioid use in the elderly with comorbidities and in patients with contraindications. This technique also enables same-day discharge and early mobilization of patients.
