Background: We present the case of a 62-year-old female presenting for resection of a colorectal carcinoma, who developed nephrogenic diabetes insipidus in the perioperative period secondary to lithium use requiring intensive care unit (ICU) admission. This case highlights the risks associated with perioperative use of lithium, as well as the clinical features and management of nephrogenic diabetes insipidus. We review current guidance regarding the management of lithium and discuss strategies for minimising risk of lithium related complications in the perioperative period.

Methods: Chart review and direct involvement with patient care.

Results: Review of clinical course of this interesting case. Patient was ventilated in ICU before and discharging to ward.

Conclusions: Lithium extubating successfully is a commonly prescribed medicine with implications for perioperative practice. Increased awareness of potential complications, and of current guidance for the perioperative management of lithium will help clinicians to care for this cohort of patients both during surgery and at ward level post operatively.
