Background: Gastric polyps have become more prevalent in recent years with increasingly liberal use of upper endoscopy. Risk of direct malignant transformation is between 0.6% and 2.1%, and is particularly true when polyp size is >1 cm. This risk of malignancy informs the need for accurate evaluation. The British Society of Gastroenterology (BSG) published guidelines in 2019 for the endoscopic evaluation and management gastric polyps. The objective of this audit was to assess the adherence of endoscopists at Portiuncula University Hospital to these guidelines in order to develop a standardised approach.

Methods: Patients diagnosed with gastric polyps during elective oesophagogastroduodenoscopy (OGD) from January to July 2023 were identified by Hospital Inpatient Enquiries (HIPE). For each, it was noted whether polyp size, number, anatomical location, and morphology were reported. The indication, representative biopsy and photo-documentation were also recorded.

Results: Twenty reports of patients diagnosed with gastric polyps were identified and included. The most common indications for endoscopy were abdominal pain (50%) and reflux (30%). Adherence to the guidelines varied. A total of 93.7% reported taking representative biopsy, 77.8% included photo-documentation, 55% of reports included polyp number and 75% reported anatomical location. Polyp size and morphology were included in one report (5%).

Conclusions: This audit demonstrated varied adherence to recommended reporting domains for gastric polyps, with evident scope for improvement. The audit findings are limited by the small sample size, which is likely due to under-representation as a result of the search strategy using (HIPE).
