Background: The mesentery in Crohn’s disease is often grossly inflamed and hyper vascular. This makes it haemostatic division challenging. Increasingly, mesenteric excision is being considered as a technical, surgical option in Crohn’s disease. The aim of this video presentation is to demonstrate a reliable technique for safe and haemostatic division of the challenging mesentery in Crohn’s disease.

Methods: Following informed consent and ethical approval, a video recording of division of the technically challenging mesentery was obtained. The case in question involved an individual undergoing exceptional surgery for complicated ileocolic Crohn’s disease. Editing was completed with Final Cut Pro.

Results: The mesentery can be haemostatically divided using the approach demonstrated. The most essential component in this approach is suture ligation of the mesentery with overlapping, horizontal, mattress sutures. This conformation ensures that all mesenteric vasculature is enclosed in the ligature.

Conclusions: The video demonstration illustrates a technique for safe and haemostatic division of the challenging mesentery in in Crohn’s disease.
