Background: Intrauterine devices (IUD) are widely used all over the world. One of the most serious complications is uterine perforation and it is very rare for IUD to penetrate the bladder after perforation. Here we report case report of IUD migration into bladder.

Methods: A 41-year-old female who presented lower urinary tract symptoms and visible haematuria for 2 years with history of IUD insertion in 2015. Her doctor believed that the IUD had spontaneously expulsed and new IUD was inserted. Pelvic ultrasound showed linear echogenic foreign body identified in the anterior bladder. CT scan demonstrated a hyper dense foreign body within the bladder measuring 3.8 cm in length.

Results: Her foreign body was removed by cystoscopy without any intra-operative and post-operative complications. Total theatre time was 20 min with estimated blood loss of 0 mL. He was discharged on post-operative day 5 after the removal of urethral catheter. Subjectively he was asymptomatic after procedure and very happy with the outcome.

Conclusions: Patient with IUD should be suggested to check the device regularly and those who with a missed IUD have to rule out the possibility of IUD migration. For patient with IUD who present with lower urinary tract symptoms, it is necessary to be aware of IUD may perforate the bladder.
