Background: The incidence of lateral pelvic sidewall involvement in magnetic resonance imaging (MRI) imaging of rectal cancer is estimated to be 12%. Lateral pelvic lymph nodes are associated with local recurrence and poor overall survival. This video presentation discusses the relevant literature and displays the surgical approach to selective lateral pelvic lymph node dissection.

Methods: A 66-year-old lady presented with altered bowel habit and bleeding per rectum. Endoscopy revealed a biopsy proven moderately differentiated adenocarcinoma of the low rectum. MRI staging revealed a 2 cm lateral pelvic lymph node. A laparoscopic anterior resection was performed followed by a lateral pelvic lymph node dissection through a robotic approach.

Results: The patient recovered well and was discharged without complication. Her surgical pathology was discussed at a subsequent multidisciplinary meeting.

Conclusions: Lateral pelvic lymph node dissection reduces locoregional recurrence and is selectively an important strategy in the management of rectal cancers with pelvic sidewall involvement.
