AB180. SOH22ABS229. Surgical management and post-operative functional outcomes of patients with penile fracture: a retrospective review of a single institution’s experience over 10 years

Background: Immediate surgical intervention has been shown to yield good clinical outcomes in patients with penile fractures. We report a single institution experience of this relatively uncommon injury.

Methods: Theatre logbooks were manually screened, between 2011 and 2021, to identify patients who underwent penile exploration for suspected fracture. Patients found to not have a fracture intra-operatively and patients with missing clinical data were excluded from the analysis.

Results: After exclusion of 2 patients due to missing clinical data, we identified 15 patients that underwent surgical exploration, 11 of which having a confirmed penile fracture intra-operatively. ‘Aubergine sign’ was present in all patients at initial presentation. Trauma to the erect penis during sexual intercourse was the most common mechanism of injury (9/11, 81.8%). Of the 11 patients, 6 had unilateral injury of one corporal body, 3 had injury of both corpora cavernosa, 1 one had a buck’s fascia tear, and 1 had involvement of both corporal bodies as well as corpus spongiosum. There were 4 cases of associated urethral injury identified and repaired at the time of surgery. One of these patients later required a formal urethroplasty due to stricture disease and urge incontinence. All patients had preserved erections post-operatively, however two reported erectile dysfunction and were prescribed phosphodiesterase inhibitors.

Conclusions: Results of our cohort show an excellent erectile function outcome following surgical repair of penile fracture. Four patients had a urethral injury evident, one of whom required a formal urethroplasty at a later date.
