AB057. Anatomical photo-documentation during oesophagogastroduodenoscopy (OGD): a snapshot in time

Background: Oesophagogastroduodenoscopy (OGD) is seen to be the diagnostic gold standard investigation for pathologies of the upper gastrointestinal tract. However, lesions may be missed due to inadequate technique and failure to visualise all mucosal surfaces. At present, there is no agreed global photo-documentation protocol. In countries with high incidence rates of upper gastrointestinal pathologies, 20–40 photos are taken at OGD. The British Society of Gastroenterology (BSG) guidelines, published in Sept 2017, recommend photo-documentation of eight anatomical landmarks during OGD; upper oesophagus (UO), lower oesophagus (LO), fundus (F), gastric body (GB), incisura (IN), antrum (A), duodenal bulb (DB), distal duodenum (DD).

Methods: To investigate the adherence to BSG photo-documentation guidelines in our unit. Retrospective review of OGD reports. Inclusion criteria: all elective OGDs with electronic reports performed between January 1st and March 31st 2019. Exclusion criteria: emergency, therapeutic OGDs. Recorded data: patient demographics, endoscopist specialty, indication, number of photos, anatomical sites photographed, pathology, pathology identified and whether photographed.

Results: A total of 303 OGDs were reviewed. Medical teams performed 121 (39.9%), taking 4.65 photos on average and surgical teams performed 182 (60.1%) taking 4.06 photos on average. Males comprised 54.46% of the cohort with the average age being 58.8 years. The most common indication was “abdominal pain” (18.8%), followed by dysphagia (14.2%) and anaemia (12.9%). Percentage photo-documentation of recommended landmarks were; UO 28.1%, LO 60.4%, F 77.6%, GB 23.1%, IN 3.6%, A 68.6%, DB 58.7%, DD 82.5%.

Conclusions: Currently our unit is not meeting requirements for photo-documentation. An education program to increase adherence will be undertaken and performance re-audited.
