Background: Epiphrenic diverticulae are rare saccular protrusions of the oesophagus at a point immediately above the lower oesophogeal sphincter. They are a subtype of esophageal diverticulae and are formed due to motility a disorder leading to a formation of a pouch through the oesophogeal wall.

Methods: Here we present a case of a 59-year-old male who was seen in the outpatient clinic complaining of severe dysphagia to liquids and solids.

Results: Following endoscopy, manometry and barium swallow testing, a large epiphrenic diverticulum was identified and progression to surgery was taken.

Conclusions: This case presentation demonstrates endoscopic and radiological findings leading to this diagnosis of epiphrenic diverticulum along with interesting intraoperative images demonstrating surgical resection of the diverticulum. We also present an up to date literature review highlighting current management options.
