Background: This article describes a case of adenoid cystic carcinoma (ACC) in the hard palate with invasion of inferior and middle nasal turbinates and maxilla anteriorly in an 82-year-old woman. Approximately 12-month-old lesion. There is no significant medical background. The patient is a non-smoker and non-drinker.

Methods: Clinical examination aided by computed tomography (CT) scan findings to assess the size and the extent of the lesion, and to check the evidence of antral involvement. Final diagnosis based on histopathological findings. Magnetic resonance imaging (MRI) investigation used to assess perineural invasion.

Results: CT investigation found large heterogeneous expansile soft tissue mass arising superiorly from hard palate with invasion of inferior and middle nasal turbinates and maxilla anteriorly. There was no involvement of maxillary sinus. No significant lymphadenopathy found. Normal vascular neck vessels. Parotid and submandibular glands were normal. Treated by surgical resection of ACC with Le Forte I osteotomy and placement of obturator.

Conclusions: ACC is a common salivary gland malignancy, frequently showing recurrence and neural invasion. Its histopathological variety helps its grading and predicts prognosis.
