Background: Herein we present a case of treating lumbosacral transitional vertebrae (LSTV)-related pain with ultrasound-guided steroid injection. LSTV, a congenital spinal anomaly, often leads to chronic low back pain and nerve impingement, posing challenges for effective management.

Methods: A 44-year-old patient with confirmed LSTV on magnetic resonance imaging (MRI) and persistent low back pain was treated with an ultrasound-guided steroid injection targeting the affected area. Ultrasound guidance allowed for precise localization of the transitional vertebra and nerve roots to ensure accurate delivery of the corticosteroid. The procedure was well tolerated, and follow-up was arranged to assess efficacy.

Results: The patient reported significant pain reduction post-injection. Functional mobility improved, and no complications were observed. Follow-up at six weeks indicated sustained pain relief and enhanced quality of life.

Conclusions: Ultrasound-guided steroid injection offers a minimally invasive, effective treatment option for managing pain associated with transitional vertebrae. This technique provides precise targeting, improves patient outcomes, and minimizes the risks associated with traditional blind injections. Further studies are needed to establish long-term efficacy and optimal treatment protocols.
